d) Upon booking an appointment, you will be emailed a questionnaire which will ask for details about your health
1.2. This information is collected by Strength & Vitality for the purposes outlined below:
a) The Enquiry Request Form collects your name and personal email address for the purposes of contacting you for your request
b) Log data collected about your website viewing helps Strength & Vitality to better understand how visitors use our online services and provides valuable information about how online services can be improved
c) Information collected for online booking requests will be used to manage your appointments
d) Strength & Vitality issues a health questionnaire by email once a booking has been made via the website. The purpose of this questionnaire is to compile important information about your overall health and is used by your practitioner to assess potential improvement opportunities and the concerns and health objectives of the patient. A pre-assessment of this information allows your practitioner to better direct your initial appointment and develop your treatment protocol.

2.1. Personal information in the way of appointment notes, supplementary historical records and results will be recorded against your patient file and will be protected by means as outlined under Clause 3.

3.1. Strength & Vitality understands that it is of upmost importance that your personal information is protected at all times and it is committed to ensuring your privacy. Strength & Vitality prioritises your privacy by:
a) Using your personal information for the purposes for which it was collected, in accordance with Clause 1.2.
b) Personal information is not shared with third-parties, except in circumstances where it is required by law
c) Information is only stored when it is required to provide ongoing health services to you as a patient. What will you do with information if the patient ceases? Consider removing if not required
d) Strength & Vitality agrees to secure data and record destruction from a period not earlier than from the time you cease to be a patient of Strength & Vitality
e) Stored information is protected by system controls that allows access to your practitioner only. Similarly, data backups are protected via the same system controls
